By definition, true breast ptosis is considered to exist when the areola–nipple complex is located below the inframammary fold. Women with breast ptosis have an excess of skin compared to the underlying breast volume.

Normally, in these cases, the skin is thin, lacks elasticity, and the presence of stretch marks is common. Due to the thinning of the ligaments that support the breast, the glandular tissue shows significant mobility over the chest wall.

It is worth noting that breast sagging is independent of breast volume, and there may be breast ptosis with small volume or with breast hypertrophy.
