Today, secondary or reconstructive surgeries are performed very often. Normally, this type of surgery has to be done because of a poor result from a previous surgery.

One of the problems in secondary rhinoplasties is the so-called “inverted V”. It is recognized because the patient, from a frontal view, presents two depressions similar to an “inverted V”

What causes it?

  • Lack of continuity between the bone and cartilage portions
  • Excessive resection of the cartilage portion
  • Excessive filing of the dorsum, which causes tearing.

How do we improve the problem?

  1. By making a graft that expands the dorsum
  2. Covering the dorsum with a synthetic sheet.

Factors to take into account in any Rhinoplasty

  • Shape and size of the nasal cartilages
  • Condition of the nasal bone
  • Skin type; thin skin allows more to be seen than thick skin.

It is estimated that one in ten patients needs secondary surgery.
